What a Certificate of Destruction Proves
A Certificate of Destruction (COD) is the document that turns "we disposed of it" into "we can prove exactly what was destroyed, how, and when." It is the record an OSHA, DEA, EPA, or state inspector asks for, and the one a facility keeps on file to close the loop on every regulated stream that leaves the building. A pickup receipt shows a container left your dock; a COD shows the waste inside it was destroyed to the required standard. A complete COD identifies the generating facility, the vendor and their credentials, the date and method of destruction, and a description of the material destroyed. For controlled substances rendered non-retrievable under DEA 21 CFR 1317, it ties back to the DEA Form 41 that documents the destruction and, where a Schedule II transfer was involved, the Form 222 records behind it.
